Meeting called to order 0833. Minutes of April 12th meeting read and adopted.

SAFETY ACTION PLANS

Field Ops - Site Inspections to be scheduled - possibly first week of June.
- Away From Home Safety Meetings - scheduled for May 22/23/24.

E/S - Goals #1 - Reduce back injuries
#2 - Riser Inspection
#3 - Reduce Injuries re proper use of tools
#4 - Reduce Track Protection Incidents
#5 - Company Policy & Procedures
Most are complete - or near completion.


UNFINISHED BUSINESS:

Item 0103-01: Letter received from L/E P. Mason concerning route indicator for intermediate signal, Notch Hill.
Action (0105) S&C (G. Mullally) investigating costs. MOTION TO CARRY

Item 0103-02: High Visibility Apparel - many Safety Hazard Reports submitted concerning vests becoming caught on equipment.
Action (0105) Committee recommended changes sent to senior H&S Committee to review.

Item 0103-04: P. Bede inquired re Laurie Shed repeaters for locotrol.
Action(0105) S&C Supvr S. Peters is investigating problems.

Item 0103-05: P. Bede inquired possibility of increasing speed restriction for new potash cars from 15 mph to 20 mph.
Action(0105) Issue elevated to senior E/S management.

Item 0104-01: B. Gardali reported a possible oil spill/puddle located middle No2. track Field.
Action (0105) B. Anderson advises area inspected and advise no evidence noted of foreign contamination under the surface.

Item 0104-02: B. Gardali requested whistle post to be installed M.95.1 Mountain Sub account people in area taking pictures.
Action (0105) TMS Palasz advises whistle posts cannot be installed - private property signs have been installed.

Item 0104-03: K. Jones advised re spill response containers are not marked for tare weight. Requested that they be weighed and decal applied.
Action(0105) TMS Palasz advises this would be a difficult task to maintain as these containers hold various products of varying weight and the weight content would not be consistent. Suggested the drivers make themselves aware of contents and also suggest taking products needed out of the containers for transport.

Item 0104-04: K. Jones requested mile boards be installed for BTMF truck crossings - he advises for gangs, etc on property that are not familiar with location.
Action (0105) Committee discussed and recommended that setoffs are shown in Timetable and carried in orientation track profiles. Also it was suggested that this could be handled on a local basis where the TMS or TMF could mark set off locations using existing signs.

Item 0104-05: G. Meloro questioned required safety precautions for equipment coming on property (rail grinder, etc) re water supplies, fire suppression for upcoming dry conditions (fires)
Action(0105) TMS Palasz advises Contractors are advised what fire protection supplies they are required to carry, as well, they will be inspected by TMS for compliance.

Item 0104-07: P. Bede requested extension of roadway adjacent to N.Track Golden to allow easier access for fuelling truck.

SAFETY HAZARD REPORTS (Incomplete)

SHR0012-02: Sicamous Yard - holes left in ballast from ballast regulator.
Action (0105) Area fixed up.

SHR0012-04: Notch Hill - west leg of wye - numerous tripping hazards on ground and scrap ties creating restricted clearance.
Action (0105) Cleanup reported underway.

SHR0012-10 Unit 9561 battery overcharging - GE advises not to shut down engine.
Action (0105) Requested clarification from Mechanical Services.

SHR0102-03: Lead locomotives (9000 series) - seat uncomfortable
Action (0105) Mechanical Services to advise; some new seats have been installed - J. Galloway to obtain list of changed out seats

SHR0102-06: Moberly back track - derail installed at west end - should also be installed east end.
Action (0105) TMS Sadek to install derail.

SHR0102-07: Grinding frogs - recent findings have determined that Manganese, a substance found in frogs, is contributing to Parkinson disease like symptoms.
Action (0105) On site testing complete - waiting for results.

SHR0102-08: Salmon Arm Station basement - asbestos warning sign to not disturb - but there is a furnace in the same location - determine if an air intake/or out-take. Also, mold in basement.
Action (0105) Basement has been cleaned - asbestos sealed.

SHR0104-01: Employee Parking Lot - Revelstoke complaint re yearly condition of drainage.
Action (0105) TMS Palasz advises Contractor waiting for dry weather to grade/spray.

SHR0104-03: Unit CEFX 134 - bad draft from under control desk when unit moving and Conductor's window open.
Action(0105) Elevate to General Motors for investigation.

SHR0104-06: Connaught Tunnel - both sides hazardous walking area.
Action(0105) TMS Sadek advises ditch area has to be maintained to allow water to drain. Committee to inspect on Site Inspection.

SAFETY HAZARD REPORTS (New)

SHR0105-01: Loss of DB on heavy trains - if DB fails or load fails for 3 DB brakes, 20,000 tons, 142 cars and 4th DB bad order - not working in load until Golden.
Action(0105) While having full DB with the four units is desirable, in the advent of a failure enroute of DB this results in a change of operating the train. An example of slower speeds commencing a downhill grade at key locations. DB failures are in fact the DB not being cut in from the initial locomotive set up location. This is an operational issue. Verification of a crew to crew form, discussion with an incoming L/E and inspection the remote locomotives at crew change locations are options.

SHR0105-02: Hot Box Detector M.118.5 Shu interrupted switching operation in Molycop spur resulting in missing instruction to stop.
Action(0105) S&C Supervisor A. Biggs advises cannot reduce wattage. Crew may have to change channels for switching purposes.

SHR0105-03: Unit 9511 very drafty, all cabinet doors need weather stripping replaced, including latches on circuit breaker door. Unit rattles bady from throttle 5-8.
Action(0105) Unit repaired.

SHR0105-04: Revelstoke Station basement, protruding hazard (improper door knob) on fire door beside meeting room #2.
Action(0105) Door has been repaired.

SHR0105-05: Safety Hi-Vis Vest caught on hinge pin, side panel door on 9100 series locomotive.
Action(0105) Employee reminded to be aware of catching hazard. Committee has sent recommended changes for safety apparel to Policy Committee.

SHR0105-06: Revelstoke Station/booking in room door - no doorknob.
Action(0105) Not considered a hazard - has passed fire inspections. There is ample access to booking in room.

SHR0105-07: Unit 9542, heater sidewall when turned on, emits odor and dust.
Action(0105) These heaters have always given an odor when turned on. Opening of the window alleviates the dust odor. Regular housekeeping of a locomotive, ie cleaning up of refuse discovered in these heaters should be discarded properly. Advising the Central Locomotive Specialist enroute will have the unit booked for any problems that arise within the cab.

SHR0105-08: Cars at west end of 4-5-6 tracks Golden. Cannot see them because of lights from the City.
Action(0105) T. Bompas advises Golden Yardmasters/Yard/Road crews have been instructed to ensure cars are pushed to the east end of these tracks.

REPORTS OF SUB-COMMITTEES:


MacDonald Tunnel - reported clean/OK. Track Programs will be commencing soon.
Hot Box Detector Alarm System Readouts - Nothing new to report at this time-still investigating lowering volume.
Health & Wellness Fair - T. Bergstrom reported due to budget cuts there will be no CP Police involvement, OHS advise they will not be able to support flu shots or cholesterol clinics. Committee discussed and it was agreed they will proceed with planning of event as planned. Invitation letters are continuing to be sent out, with many interested parties planning to
be involved. Clarence Thacker (EFAP) advised he believes this to be a very beneficial venture for all concerned.

DISTRICT SAFETY & HEALTH ACTION PREVENTION COORDINATOR REPORT

T. Bergstrom advised committee of inspection of Field Bunkhouse by Environmental Inspectors and contractors re determining proposal for removal of asbestos.

Orientation Tunnel Procedures E/S - complete/ went very well.

NEW BUSINESS:

Item 0105-01: P. Bede advises Williamette crossing and airport (Golden) has whistle posts missing.
Action(0105) TMS Sadek to advise.

Item 0105-02: P. Bede reported back track switch east end Moberly hard to throw.
Action(0105) TMS Sadek to advise.

Item 0105-03: K. Jones reported possible hazard at Bell Pole where there is a chain gate difficult to see.
Action(0105) TMS Palasz reported gate will have reflectors/marker applied.
