SAFETY ACTION PLANS

Field Ops - 4 hour safety meetings complete. Monthly site inspection complete.
E/S - 2001 Safety Program (E Palasz) 100% complete

NEW BUSINESS

Item 9912-02: J. Wells requested washer/dryers be provided for means for cleaning work
clothes
which have been contaminated. (Referred to Canada Labour Code COSH9.44(4))
Item elevated to ES System H&S Policy Committee.
Action(0112) Washer/dryer has been installed at Salmon Arm - J. Wells reported being
used on a regular basis. Awaiting further installation at Chase, Kamloops, Revelstoke, Golden. Monitor MOTION TO CARRY


Item 0103-04: P. Bede inquired re Laurie Shed repeaters for locotrol.
Action(0112) Repeaters/meeting rescheduled for sometime in Jan/02. D. Zatko to
monitor.

potash cars from 15 mph to 20 mph.

SAFETY HAZARD REPORTS (Incomplete)

SHR0110-04: Exhaust fumes - diesel shop.
Action(0112) Further modifications to fans done. Work procedures being developed.

SHR0102-03: Lead locomotives (9000 series) - seats uncomfortable
Action (0112) Mechanical Services advises some new seats have been installed. Units
should not be leaders but continue to be. Committee has exhausted all avenues,
recommend elevate to Policy Committee. J. Galloway to follow up.
MOTION TO CARRY

SHR0111-02: Dusting suppressant overspray compound on outside of coal cars
making for extremely dirty and slippery when wet (grabirons).
Action(0112) No complaints received from Golden. Peter Lanyi, Manager
Coal, advises spray should be confined to the width of the car avoiding overspray off
the side walls, however spraying of the front and rears is unavoidable at this
time since turning the spray on and off to avoid is not possible at 10mph. Problem will
be taken into consideration in the design of a permanent facility. No spraying
will take place for temperature below 0 Cel. Bulletin have been issued regarding
slippery surfaces.

SHR0111-05: Golden taxi driver drove erratically with crew.
Action(0112) Taxi Exception Reports should be completed for these instances.

SHR0111-06: Field Station rain eavetroughs plugged and dripping onto
sidewalk and freezing making icy conditions for walking.
Action(0112) K. Jones advised eavetroughs replaced and heat tape applied.

SHR0111-09: Lack of clear Company policy with regard to hazards created by
trains stalling, specifically during winter conditions.

SAFETY HAZARDS REPORTS (New)

SHR0112-01: Train with EGT 17,643 and only 2 dynamic brakes. Second
engine had b/o DB tagged by shops.
Action(0112) Policy states minimum DB factor 16 (this instance met
policy). W. Hayman requested that distributors should make effort to change out locomotives.
D. Zatko to investigate.

SHR0112-02: Unit 5772-table attached to wall broke away leaving sharp
point towards Conductor.
Action(0112) Unit reported for repair

SHR0112-03: Spray bottles with cleaning compound for cleaning units.
Action (0112) D. Zatko agreed to supply cleaner - employees can purchase
spray bottles.

SHR0112-04: Door to booking in room sill loose, tripping hazard
Action(0112) D. Kermack advises sill was bent, has been removed and is
not necessary. Screws have been tightened, door now opens and closes with no
problem.

SHR0112-05: Condition of ballast and ties Revelstoke wye.
Action(0112) Tie program scheduled for 2002 - wyes will be looked at.

SHR0112-06: Smoking in locomotives and booking in rooms at Kamloops and
Field
Action(0112) Signs posted. Employees must report incidents to their
Supervisor to enable followup.

SHR0112-07: Snow conditions while operating on heavy grades - suggest
advanced clear to stop signals on all heavy grade.
Action(0112) D. Zatko to investigate costs with S. Peters.

SHR0112-08: Lack of snow removal and sand Revelstoke Station (Dec 1/2) (5
SHRs)
Action(0112) First major snowfall occurring on a weekend. Snow removal
during the week is done by a contractor (Mon-Fri). All other snow removal is
done by CPR equipment, however on this particular weekend the front end
loader had been sent to Glacier to replace the Glacier machine which had broken
down and did not return to Revelstoke until Dec 3. The yard, parking lot and
station platform has been cleaned and sanded. Sand is made available at four locations
around the station for immediate use.

SHR0112-09: No switch brooms at half the switches on Mountain Sub and units.
Action(0112) Site Inspection in November noted most switches had brooms.
Further clarification required as to exact location where brooms are required.
Notification to RTC and TMS will rectify problem. Locomotives should be reported to
Mechanical Specialists.

SHR0112-10: Release levers on most old coal cars do not work due to age.
Hand brakes have to be twisted off using full force resulting in strained back
muscles, etc.
Action(0112) Clarification with regard to specific car numbers are
required for Mechanical Services to enable repair.

SHR0112-11: R Taxi Van #4 - requires weatherstripping, reported ties worn out.

Terry advised committee of upcoming presentation course for interested
(FOPS/ES).

NEW BUSINESS

Item 0112-01: J. Meloro expressed concern with regard to lack of protection in Revelstoke Yard when contractors are on site unloading contents from tank cars.
